7th Grade Bowling and Ice Skating Permission Slip

Hey 7th graders, if you haven’t handed in your permission slip to go ice skating or bowling on March 9th, please do it soon! The deadline is Wednesday, February 15th. The cost for both trips is $10. Please print out this file and complete it, or see Ms. Haft in Rm. 333 for a hard copy. All permission slips and cash or checks should be turned in to Ms. Smith or Ms. Wong by next Wednesday, 2/15!

NYC PARENT NOTIFICATION/CONSENT FORM DAY TRIP Chancellor’s Regulation A-670

 

We are happy to announce that the 7th grade team has planned a student incentive trip on Thursday, March 9, 2017, to support positive community participation and citizenship.  Students will select between option A (Bowling at Melody Lanes) or option B (Ice skating in Prospect Park).  

 

Students should bring a bag lunch and/or money for refreshments, as no hot lunch will be available on this day.  You may request a bagged school lunch on the back of this form.

 

The cost of both trips is $10.00, but financial assistance is available.  Contact Mr. Smith at esmith@ms447.org or Ms.Haft at chaft@ms447.org, or check the box on the back.

This trip will include the following physical and sports activities: Bowling/Ice skating

 

  1. a) I understand that there are risks of injury associated with the above-listed physical and sports activities and I consent to my child’s participation in all these activities except for the following:__________________________________________________________.

    c) I agree that in the event of an emergency injury or illness, the staff member(s) in charge of the trip may act on my behalf and at my expense in obtaining medical treatment for my child.                                                                             
  2. d) I understand that my child is expected to behave responsibly and to follow the school’s discipline code and policies.
    e) I agree and understand that I am responsible for the actions of my child. I release the school from all claims and liability that arise in connection with the trip, except if due to the negligence of school officials.
    f) I understand that I am responsible for getting my child to and from the departure and return sites identified above. I understand that my child shall be accompanied by staff member(s) during the trip, including while traveling from the departure site to the destination site, and from the destination site to the return site.
    g) I understand that alcoholic beverages and/or illegal drugs are prohibited and have discussed this prohibition with my child. I understand that if my child is found in possession of these substances, he/she will be subject to school disciplinary procedures and possible criminal prosecution.
    h) I understand that students who violates the school’s discipline code may be excluded in the future by the school from participating in a trip.
